All new patients must attend an eligibility clinic. An appointment can be obtained by calling 545-5700 extension 5004.  Please leave name, daytime phone number, date of birth and if you are seeking medical and/or dental care.  Please note: we currently have a 6 month wait list. 

Documentation required at eligibility clinic:

1 – photo ID

2 – proof of social security number (social security card, tax return, W2, insurance policy, etc)

3 – proof of address (mortgage, lease, utility bill)

4 – proof of income (see income documents needed)

5 – federal tax return, if you file ( we are only accepting 2009 federal tax returns)

After your eligibility packet is reviewed, you will received your first medical and/or dental appointment.
