Our dental program offers an array of dental services to our patients. We provide preventative care including cleanings, patient education, screenings, restorative fillings, extractions, and dentures. Our dental program collaborates with our pharmacists and primary care providers to help patients better understand the connection between oral health and their overall health.

CHRONIC DISEASE MANAGEMENT
The chronic care model allows patients to be seen by a consistent team of health care providers, creating a provider-patient relationship and better chance at involvement/partnership in health management. Individuals diagnosed with a chronic condition, defined as a medical problem that cannot be cured although treatments may bring the condition under control, such as diabetes, hypertension, COPD.

MEDICATION ASSISTANCE PROGRAM
We process hundreds of applications for medication programs offered by pharmaceutical companies for uninsured and low income patients. These medications are processed by our pharmacist and dispensed to patients at scheduled pick up times.

MENTAL HEALTH COUNSELING
We know that mental health is a key part of comprehensive healthcare. We provide mental health counseling for patients dealing with minor to moderate mental health care problems so patients don't have to deal with additional barriers to accessing care.

We offer full women's annual exams, including mammograms. We do NOT provide prenatal care.
